Position: Foot Care Nurse/ Registered Nurse/Registered Practical Nurse – Full Time

Reporting to: Executive Director & Physicians (for clinical responsibilities)

The Burlington Family Health Team provides patients with care and support from complementary interdisciplinary healthcare professionals (IHP) who work together, each utilizing their experience and skills to ensure patients have access to the best possible care.  


Position Summary:

The RN/RPN play a key role in the delivery of BFHT patient programs and services.  They are a contributing member of the Quality Improvement Committee and oversee a number of programs.  The RN/ RPN programs provide patient care through chronic disease management and episodic care.  They work with the physicians and other IHP’s to provide continuity of care.



Primary Role

  • Manage the Diabetes and INR programs
  • Develop and support a Foot Care Program
  • Develop and support new programs with of focus on Chronic Disease management
  • Lead or collaborate with other health care providers for provision of group classes and specialized program delivery
  • Knowledge and experience working collaboratively with community resources
  • Applies recognized clinical practice guidelines and best practices to screening, monitoring, and assessment of patient’s physical and emotional well-being



  • Current registration as an RN/RPN with the College of Nurses of Ontario
  • Current CPR Certification
  • Demonstrated ability to assist patients, family members or other clients with concern and empathy, respecting their confidentiality and privacy and communicating in a courteous and respectful manner
  • High degree of resourcefulness, flexibility and adaptability
  • High level of critical and logical thinking, analysis, and /or reasoning to identify underlying principles, reasons, or facts
  • Proficiency in OSCAR EMR system is an asset
  • TEACH Certification is an asset
  • Certificate in Pharmacy Management of Oral Anticoagulation Therapy is an asset
  • Comprehensive understanding of patient privacy and confidentiality



  • Bachelors of Science in Nursing (BScN)/ Registered Practical Nurse Diploma
  • Foot Care Nurse Certification



  • Minimum 1 year of experience, preferably in a primary care setting
  • Experience in developing and maintaining Quality Improvement Programs
