Integrated Best Practice Supervisor for Toronto Central
1 year full-time contract position
What is the job?
This is an innovative one year pilot role reporting to the Regional Manager that will focus on integrated service delivery and providing quality oversight to clients at point of care. The VHA Sub-Region project has been developed to address home care changes stemming from the Minister of Health’s Patients First Act specifically regarding the transition of funding and service delivery responsibilities for home and community health care from the CCAC’s to the LHINS.
As the Integrated Best Practice Supervisor, you will ensure clinical competence across three streams of VHA business—Personal Support, Nursing and Rehab. Supporting other supervisors as needed, you will be flexible and knowledgeable as the role evolves but also able to provide nimble supervision as needed as part of the Toronto Central Team.
You will actively participate as a key member of the sub-Region project team – attending meetings with CCAC, LHIN and agency partners to restructure and integrate services across the project geographical area. You will report on and participate in the development of key performance indicators, measuring the success and challenges associated with the project.
Am I qualified?
The ideal candidate will have an undergraduate degree in Nursing/Science in Nursing and current certificate of competency (licensure) from the College of Nurses on Ontario . You will have demonstrated experience in working effectively with all three lines of business: Nursing, Personal Support and Rehab. You will have 5 years progressive experience in nursing of which 2 years include experience in a supervisory or advanced practice role.
You will have a solid understanding of the health care services and delivery processes in the community/home care sector. You will have the ability to model and lead a team in the provision of client and family-centered care, including client/family-centered assessment, documentation and advocacy.
You will have demonstrated commitment to the client and a family-centered approach in all aspects of your work is essential for this role. You will also have demonstrated strength in the following areas: problem solving and organizational skills, attention to detail and the ability to prioritize effectively in a high pressure environment, ability to interact professionally with staff, clients, funder and colleagues, have patience and courtesy with clients, funders.
You are a strong clinical practitioner with the flexibility and the ability to use knowledge-transfer to put evidence-based knowledge into practice across all three areas: personal support, nursing and rehab.
You must have a valid driver’s license and use of a car and you must be able to part of the on-call rotation for Supervisors.
