As part of an inter-professional team in a rapidly evolving Toronto neighbourhood, Regent Park CHC offers an opportunity for a Nurse Practitioner who is seeking a challenging, rewarding position providing comprehensive primary care services to clients who experience barriers to good health. As a member of an interdisciplinary team, working on or off site, the NP serves a high density, low income, multicultural, inner city community through a variety of methods including direct patient care, health promotion and education, outreach to marginalized individuals and groups, advocacy, and community development.
Specific duties include:
Provides direct patient care within the RNEC scope of practice.
Providing supportive and therapeutic counselling and crisis intervention on an individual or group basis on such issues as sexuality, family planning, pregnancy choices, family violence, lifestyle;
Provides health promotion and education both to individuals and to groups in a non-judgemental manner
Advocates on individual and community issues that are of concern to our clients and the community at large
Maintains an active role in outreach and community development
Required Skills Knowledge and Abilities
You are currently registered with the College of Nurses of Ontario as a Registered Nurse in the Extended Class, have demonstrated commitment to client-centered, community-based care, are a strong team player with excellent communication and listening skills. You have excellent decision-making skills in assessment, diagnosis, health care management as well as supportive counselling/health education. Ideally, you have experience working with the populations served by the Health Centre and working in the community health sector. You demonstrate initiative, creativity and flexibility in work with clients, teams and communities. You understand health and social issues from a systemic perspective and operate within an anti-oppression framework. Knowledge of the Regent Park community and a second language are key assets.
