Job Description

Job Summary

The West End Integrated Falls Prevention Program will address falls prevention strategies for seniors living in the community and their caregivers as well as best practices in relation to treatment for those seniors who do fall.  The Registered Nurse will work with a multi-disciplinary health care team in the provision of individual client falls assessment and services, and education and outreach based on best practices. 17.5 hours/week, permanent position - cost-shared healt/dental benefits, HOOPP (pension) eligibility. Job #2018-0004.   

 

Job Specific Responsibilities

  • Provides individual assessment, education and outreach to seniors; within the scope of a registered nurse including appropriate assessments and counseling, screening, referral, education, and follow-up and other clinical activities consistent with the College of Nurses of Ontario standards of nursing practice.

  • Keeps complete, accurate and timely records of client care.

  • Sees clients in the community through outreach and home visits.

  • Provides appropriate support and consultation to other health care team staff.

  • Participates in the preparation of new materials based on best practices for client education.

  • Participates in regular chart reviews and case conferences.

  • Provides health promotion activities as appropriate to the position.

  • In consultation with the Program Co-ordinator and health care team, develops policies, procedures and protocols for client management and for quality care.

  • Participates in the establishment and maintenance of a program for continuing staff education, in consultation with Centre health professionals.

  • Participates actively in information sharing within the program team and other partner agencies.

  • Acts as a health resource to staff and program.

  • Assists in the collection and analysis of relevant data for administration, planning and evaluation.

  • Works with internal and external service providers and with relevant community groups.

  • Participates in the development of primary care and health education programs based on client and community identified health needs.

  • Participates in community outreach activities, related to falls prevention.

 

Qualifications

  • Baccalaureate Degree in Nursing.

  • Registration with the College of Nurses of Ontario.

  • Minimum of four years of nursing practice in a community setting.

  • Nursing experience working with seniors, low income families and multicultural communities.

  • Bilingual (French/English) is essential.

  • Basic computer skills an asset.
