Position: Registered Nurse

Department:  The Phoenix Program, Early Intervention Service - Outpatient Mental Health

Status:  Permanent Part-Time. Must provide availability up to 0.5 FTE.

Hours: Monday to Friday, Days and Evenings - Likely 4 days a week, 1300-1730  (Subject to change as per operational needs)

Site: Oakville

POSITION SUMMARY:

Within a multidisciplinary team, the Nurse will coordinate early identification/screening, diagnostic, and intervention services for individuals experiencing first episode psychosis. The Nurse, in collaboration with the clinical team, will ensure the development and implementation of a comprehensive individualized treatment plan that mitigates the effects of a first episode of psychosis. This position includes responsibilities related to providing expertise in psychiatric nursing to clients in collaboration with the Halton Phoenix team and the broader Regional Program team. The Nurse will implement clinical best practices, engage in therapeutic partnerships with clients, their families and other community resources and will participate in community education and research. 

RESPONSIBILITIES:

  • Conduct initial assessment interviews.
  • Administer structured clinical scales.
  • In collaboration with the multidisciplinary team, the clients and their families, develop treatment plans based on client goals, knowledge of psychiatric and psychosocial treatment modalities.
  • Use recovery and rehabilitation models in assessment, planning, intervention and evaluation of client and family-centred interventions.
  • Engage in shared-care with the client and the client’s primary practitioner.
  • Provide education to clients, families and community partners.
  • Administration and monitoring of psychiatric medications including both oral and intramuscular injections.
  • Liaise and advocate between the client, family, other members of the treatment team and the larger treatment community.
  • Provide crisis intervention and support.
  • Provide care acknowledging unique needs of the young adult clients and their families.
  • Supporting clients and/or families by meeting in both the office and community locations including the client’s home and public meeting places.
  • Facilitate with the client’s/family’s consent, the involvement of significant others/social network in the assessment and goal definition process
  • Participate in team, regional and/or community based group planning, implementation and evaluation as needed.
  • Participate in program evaluation and other research and best practice.
  • Attend team, administrative/regional and other relevant system-level meetings.
  • Complete required documentation and statistical collection in a timely manner.
  • Supervise students/volunteers as required.

 

 QUALIFICATIONS:

  • Current registration with the College of Nurses of Ontario and in good standing. 
  • CNA Certification in psychiatric/mental health nursing preferred.
  • Minimum 3-5 years mental health experience with skills, knowledge, and attitudes commensurate with the provision of intensive clinical case management services and crisis intervention functions.
  • Experience in early intervention in psychosis preferred.
  • Ability to assess efficacy and side effects of many different psychiatric medications: antipsychotic medications, mood stabilizers, and antidepressants.
  • Knowledge of unique developmental issues of young adults and their families preferred.
  • Knowledge of psychosis including etiology, prognosis, treatment modalities and current psychiatric medication treatment trends required
  • Group leadership skills using an educational model, developed through a combination of experience and academic preparation.
  • Communication skills, interviewing skills, critical thinking and problem solving skills.
  • Knowledge, skills and experience in the principles and practices of psychosocial rehabilitation and recovery including hope and empowerment.
  • Ability to work in collaborative partnership with clients, family and community resources.
  • Ability to advocate on behalf of clients and families.
  • Working knowledge of the Ontario Mental Health Act
  • Knowledge of community resources to support clients and families.
  • Skills in Microsoft Outlook, Windows, Word, Excel and PowerPoint. Familiarity with accessing online laboratory results.
  • Knowledge and experience working with clients with Concurrent Disorders.
  • Ability or willingness to learn the administration of structured clinical scales.
  • Willingness to participate in research and program evaluation.
  • Evidence of continued professional development and education.
  • Valid driver’s license and access to reliable transportation required.
