 Toronto East Integrated Service Team (TE-IST) Team

 Bargaining Unit Position

One (1) Permanent Full-Time


Cota seeks a Mental Health Case Manager-Registered Nurse for our Toronto East Integrated Service Team. This intensive case management position will be part of a multidisciplinary team working with diverse populations including serious mental illness, addictions, acquired brain injury, geriatric mental health conditions and dual diagnosis.


As a member of this team, you will have an independent case load as well as being responsible to provide expertise in the areas of physical and mental health assessments and metabolic monitoring.  The Case Manager- RN is also responsible for providing oversight, consultation, education and administration of medication.



  • University Degree in Nursing
  • The ideal candidate will have 2 years direct experience in community  psychiatric nursing, however new grads are welcome to apply
  • Maintains a certificate of competence with the College of Nurses of Ontario
  • Demonstrated strong commitment to the right and ability of each person with a serious mental illness to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services
  • Familiarity with the Ontario Mental Health Act and Mental Health Reform
  • Knowledge of PSR and Recovery as guiding principles for assessing and developing treatment plans for clients
  • Demonstrated skill and competence in establishing supportive, trusting relationships with persons with a serious mental illness with respect for clients’ rights and preferences
  • Sound knowledge of mental illness, addictions signs and symptoms.  Knowledge of psychotropic medications, especially the newer neuroleptics and their intended effects and possible side effects. Provide expertise to the team and clients about pharmacological and medical issues, including evaluating, charting psychotropic medication effectiveness, complications, side effects and arranging for required blood work according to protocol
  • Ability to dispense and inject neuroleptics
  • Ability to teach clients, families, and other staff about medications, symptoms, side effects and health and wellness behaviours
  • Ability to make appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric and medical treatment
  • Knowledge of concurrent disorder and those living in unstable housing situations or homelessness
  • Demonstrated ability to work independently, cooperatively and constructively within a team environment
  • Ability to work with agencies, hospitals and the broader community
  • Knowledge of trends and issues related to the delivery of services to a mental health client population
  • Sound knowledge of community resources available to people living with mental health challenges including those related to diversity
  • Excellent organizational and interpersonal skills, and the ability to work flexible hours, including weekends and holidays if required to do so
  • Excellent computer skills (e.g., Word, Excel, Microsoft Outlook)
  • Ability to manage and adapt positively to an ever changing health care/work environment
  • Effective oral and written skills
  • Experience working in a community agency an asset



  • Visits will be conducted in the clients’ homes, hostels, hospitals and other community settings.
  • Weekend and evening rotation may be required.
