The primary care RN collaborates with members of an inter-disciplinary health team to plan, implement, coordinate and evaluate primary health care for clients in consultation with clients and their families; performs primary care physical assessments, triage, health education/counseling, history taking, and other clinical activities; and develops and/or initiates health teaching, screening programs, and workshops by identifying health needs of clients and various other community groups.
- Undergraduate degree in nursing from a recognized university or combination of nursing diploma and additional relevant courses/training
- Current registration with the College of Nurses of Ontario
- 3-5 years nursing experience in a community setting or combination of community, hospital, and/or public health settings; experience in a primary care setting an asset
- Smoking cessation certificate preferred
