Job Description

At Scarborough and Rouge Hospital (SRH), the patient experience comes first.  With three hospital sites (Birchmount, General and Centenary) and five satellite sites, SRH provides a broad spectrum of health services to one of the most diverse communities in Canada.

Created through a merger of The Scarborough Hospital's Birchmount and General sites and Rouge Valley Health System's Centenary site in December 2016, SRH is committed to delivering the highest quality patient- and family-centred care, with a focus on enhancing access to services for the Scarborough community.

Scarborough and Rouge Hospital is committed to Patient and Family Centred Care (PFCC) and Lean thinking as fundamental philosophies underpinning our transformation efforts. Reporting to the Manager of Strategy and Planning within the Innovation and Improvement Office, this position will play a key role in supporting multiple clinical integration activities of SRH throughout the project cycle, with an initial focus on clinical standardization work across our three main hospital sites. The Improvement Facilitator (Clinical) - Integration will collaborate with clinical leaders, front line teams and patient/family advisors. He/she will advance clinical standardization and integration through development of policies, protocols and tools; application of process improvement, quality improvement and change management methodologies; stakeholder engagement; and group facilitation and coaching. All of the integration work will be done through a patient experience lens. The Improvement Facilitator (Clinical) - Integration brings clinical expertise and strong facilitation skills that will strengthen corporate capacity in change management and quality improvement as we work toward our realization of the benefits of the integration as outlined in the SRH Value Realization Framework. Note that this is a two-year contract with possibility of extension.

Responsibilities:

  • Fosters a culture of continuous improvement by guiding and supporting staff and clinicians in undertaking integration related improvement activities using Lean principles and tools, with a focus on standardization of structures, tools and processes.
  • Effectively applies PFCC and Lean principles and tools in a variety of clinical settings and to a variety of standardization initiatives. Examples may include the standardization of clinical order sets and pathways; standardization of clinical policies; establishment of committees; and supporting physician-driven integration opportunities.
  • Facilitates the completion of all components of change strategies including staff education plan, communication plan, and development of metrics tracking tools, etc.
  • Develops project plans that define scope, objectives, timelines, deliverables and alignment with integration goals in collaboration with senior management and stakeholders.
  • Provides project management expertise to support the execution, management, implementation, monitoring and reporting of clinical integration initiatives which span various functional areas.
  • Demonstrates systems thinking by considering the potential impact of process changes on upstream and downstream processes, overall functioning of the hospital and external health system partners.
  • Provides guidance, direction and support to the team in the establishment of sustainability mechanisms.
  • Participates on the hospital's Order Set Steering Committee and on order set development teams as appropriate.
  • Participates on other committees as required.

Qualifications:

  • Undergraduate degree.
  • Master's degree in a health related discipline, business or industrial engineering, or Improvement Advisor certification through the Institute of Healthcare Improvement.
  • Current registration with a health professional regulatory body and/or be a member in good standing with a professional association preferred.
  • Minimum of 3 years of recent experience in a clinical or quality improvement role and/or active involvement in leading quality improvement or transformation initiatives at the corporate level preferred.
  • Strong working knowledge of, and experience with, Lean management principles and Lean tools
  • Strong working knowledge of, and experience with, PFCC and patient safety
  • Demonstrated experience managing and executing complex change initiatives
  • Project Management certification and/or experience an asset
  • Highly developed judgement skills combined with tact, diplomacy and a proven ability to foster and maintain effective working relationships with a wide range of diverse stakeholders including physicians, management, frontline staff and patients/families
  • Evidence of leadership skills and coaching skills
  • Expertise in group facilitation techniques
  • Excellent interpersonal skills with a participative/consultative style that facilitates consensus, collaboration and team work
  • Excellent verbal and written communication skills
  • Demonstrated ability to work independently and as part of a collaborative interprofessional team
  • Excellent organizational and time management skills
  • High level of proficiency with Microsoft Office applications including Word, Excel, and PowerPoint
  • Demonstrates a commitment to customer service when dealing with patients, colleagues, volunteers and other members of the community and public.
  • Demonstrated pattern of good attendance
  • Ability to travel to all sites across the organization
  • Demonstrates values consistent with SRH's Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
  • Utilize protective equipment provided and identify and reports hazards
  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
  • Excellent work performance and attendance record
