Nurse Case Manager

The Canadian Mental Health Association Toronto Branch is a non- profit, community based mental health agency providing a wide range of services across Toronto. The Mental Health and Justice Case Management Team (MHJCM) is being enhanced to better support individuals with complex mental/physical health and substance use issues from a recovery- based philosophy. With a dedicated CD Specialist and Nurse Case Manager the team is becoming a multidisciplinary one that will provide individualized supports to improve mental health and physical well- being, and to increase housing tenure.  Service is provided in collaboration with the Mental Health and Justice Case Management Team as part of a city wide mental health and justice initiative.


Job Description

The Nurse Case Manager will provide community- based support one to one with clients, facilitate groups and make linkages to other services. The Nurse Case Manager will also provide individual and team consultation on mental/physical health issues to related justice/forensic programs; this is a key element of the role.  Case load will be determined based on intensity of service dedicated to RN assessments and interventions.

Responsibilities include:

  • Client-centred engagement, service provision and interventions; monitoring, evaluation and follow-up

  • Developing therapeutic alliance with clients and their support systems/significant others

  • Assessment of mental and primary health conditions; orientation to program and agency

  • Providing oversight, direct service, consultation and education around mental/physical health conditions, medications, side effects and symptoms to clients/colleagues

  • Monitoring treatment plans, medication administration and coordinating follow up/referrals

  • Consultation to other staff on physical/mental health issues and available treatments

  • Crisis prevention and intervention; skill development and goal planning

  • Providing resources, rights information and advocacy;  work from a recovery- based perspective

  • Supporting the agency’s primary health care and chronic disease management initiatives

  • Accurate, timely documentation


    Qualifications: The successful candidate will possess the following:

  • BScN Degree and Registration with the College of Nurses of Ontario

  • 3 years of experience working with people with mental illness, preferably community-based

  • Excellent skills in mental/physical health assessment and teaching, medication monitoring, motivational interviewing, case management and crisis prevention/intervention

  • Work experience and in depth knowledge of concurrent disorders, homelessness, recovery principles and the criminal justice system as it relates to mental health

  • Knowledge of legislation including PHIPPA and the Mental Health Act an asset

  • Ability to work both independently and collaboratively in a dynamic team environment;

  • Strong organizational/time management skills; strong self care practices

  • Excellent communication, listening, and interpersonal skills

  • Ability to consistently demonstrate an anti-oppression approach to practice

    A valid Ontario driver’s license is required.  Successful applicant must have access to a vehicle and valid insurance for workplace use.
