RAI-MDS Coordinator

Position Purpose
Reporting to the Director of Care, is responsible for coordinating and implementing best practices in the Resident Assessment Instrument - Minimum Data Set (RAI-MDS) program in the Region's LTC Homes; providing support for accurate RAI applications, assessments, audits, quality improvements and data submission processes, and staff education.

$32.22 - $35.03 per hour. This is a Union position.

  • Successful completion of a Community College Diploma in Health Sciences or related field or approved equivalent combination of education and experience.
  • Current regulated or licensed health care practitioner (e.g. RPN).
  • Minimum one (1) year in a Long Term Care setting with clinical experience including restorative care programming and using RAI-MDS, project leadership, and experience with adult education strategies/facilitation.
  • Experience in making presentations to groups.
  • Demonstrates strong initiative, critical thinking and enthusiasm to collaboratively support the development of interdisciplinary care programs and other initiatives
  • Satisfactory Police Vulnerable Sector Checks (PVSC).
  • Knowledge of Long Term Care or health care policies on assessments and care planning processes.
  • Knowledge of and demonstrated ability in corporate core competencies including communication, innovation, teamwork and collaboration, and personal ownership.
  • Computer literacy utilizing MS Office software applications and Point Click Care (PCC) system
  • Strong problem solving skills, aptitude for detail oriented work and enthusiasm for RAI-MDS program.
  • Good interpersonal and communication skills to work within a diverse community and ability to deal empathetically with residents, family members, etc., to contribute to their well-being.
  • Proven ability in setting and meeting objectives and leading new initiatives.
  • Ability to work effectively independently or as a part of a team.
  • Ability to travel to off-site locations in a timely and efficient manner, as required.
  • Ability to work outside regular business hours, and work shifts and weekends, as required.
