To provide effective case management and care coordination for clients and their families to ensure the implementation and ongoing effectiveness of quality home care services and client satisfaction.
-Performs initial client assessments
-Develops a plan of care based on the assessment, and effectively coordinates with the CCAC, other agencies, and family members to ensure the client is receiving the support they need.
-Determines the most appropriate caregiver for the client based on skills and client requests.
-Provides health promotion and education for both the client and the family.
-Provides ongoing updates to the client and the family.
-Communicates with other health care professionals and service providers to ensure continuity of care for the client.
-Participates in hospital discharge planning meetings.
-Responds promptly to client questions via telephone or email.
-Makes home visits on regularly scheduled and on an as-needed basis to ensure the plan of care is up to date and meeting the needs of the client.
-Provides health care system navigation assistance and coaching for clients and their families
-Provides timely updates to the family physician or nurse practitioner
-Monitors client vital signs, glucose, and weight remotely using the technology available.
Supervision: Ensures the delivery of safe, competent and compassionate care by personal support workers, companion caregivers, and RPN’s.
-Recruit and interview candidates.
-Conducts reference checks.
-Prepares letters of offer and other paperwork for selected candidates.
-Conducts home visits to monitor field staff skills, performance and client interactions during the probationary period and as appropriate thereafter.
-Maintains flexibility for responding to impromptu and scheduled home visits with field staff.
-Provides constructive feedback to field staff as necessary.
Practice Support: Ensures adherence to RNAO best practices
-Demonstrates clinical expertise by applying theory and knowledge of community nursing practice and acts as a resource to staff to ensure continued skills enhancement.
-Provides assessment, support and problem solving strategies focused on the achievement of quality outcomes for the client.
-Ensures implementation and adherence to all contract requirements.
-Represents CalaCare on committees, task forces and projects related to community health care.
-Supports development of staff skills and knowledge through an effective education plan that includes in-services, take away learning opportunities and off site education.
-Delegates nursing tasks appropriate to unregulated staff (i.e. home glucometer use, subcutaneous injections, non-sterile dressing changes).
-Respond to questions from front-line staff via phone and email
-Accesses additional appropriate community resources to support clients and families to remain safely in their homes.
-Investigates and responds to complaints on unacceptable clinical practice immediately.
-Assists with scheduling for short notice and urgent shifts.
-Informs and educates front line staff and administrative assistant on operational changes and ensures effective implementation.
-Develops relationships with referral partners, such as hospital discharge planners, long-term care facilities, retirement homes, and family physicians.
-Attends community health and business events to promote CalaCare and increase awareness of the services provided.

-Must be licensed to practice as a Registered Nurse in the province of Ontario
Minimum of 2 years clinical experience.
- Emergency Room experience is a definite asset
- Long-term Care experience is an asset
- Community nursing experience is an asset.
- Must be a quick learner with a drive to succeed.
- Must be tech-savvy and interested in how technology can improve client care.
-Up to Date CPR/First Aid
-Driver’s license and reliable vehicle, with a willingness to travel within the GTA
-Vulnerable Sector Police Check
-Up to date immunizations
-On-Call duties 7-14 days per month for urgent caregiver or client issues.
-Days of work are Sunday- Thursday, with Friday and Saturday off.
