The Sunnybrook Academic Family Health Team (SAFHT) serves ~ 9500 rostered patients in North East Toronto and is physically located on the border of the Toronto Central and Central LHINS on the campus of Sunnybrook Health Sciences Centre. Thirteen academic physicians, six nurses, two social workers (1FTE), .5FTE pharmacist, .4FTE dietitian, .8FTE care navigator and .5FTE occupational therapist all provide care and contribute to the unit's academic manate by participating in the education of family medicine residents, medical students and interprofessional health care learners. Although the Family Practice Unit has been in existence for over 45 years, the SAFHT became operational in the fall of 2011. The SAFHT encorporates a Quality Improvement lens on all its activities and develops and submits a Quality Improvement Plan annually. Staff participate on committees including Quality of Care, Patient Safety, Patient Engagement and Education. "Doing it Better" Rounds are held by monthly. Programs include Adolescent Outreach, Homebound Seniors, and a full Diabetes Education program. Group education sessions include Heart Healthy classes, Choices and Change, Aging Well, Chronic Disease Management etc.  The SAFHT Strategic Plan can be viewed on our web site.

 

 


Job Description

Overview:  The registered nurse , working to his/her full scope of practice provides professional nursing and collaborative care to clients and families of the Academic Family Health Team (FHT) across the life span and health/illness continuum. Nursing care includes health promotion activities, disease prevention counseling and assisting the client with chronic disease management. Ensuring adherence to FHT policies and nursing procedures, standards and practices, the RN is able to autonomously meet the nursing care needs of clients whose needs are not well defined/established or are changing. The RN provides clinical support to the physician, clients' follow-up for  education and referrals, immunizations, dressing changes, telephone advice and triage and wound care. The RN also contributes to the education of health care learners. In this position the RN works with 2 experienced family physicians (one of which includes obstetrics in her practice) and is able to focus his/her care on those clinicians' practices. The RN also works collaboratively with other interprofessional health care providers and support staff to provide optimum care.
