Hospice Georgian Triangle's (HGT) Clinical Services Coordinator (CSC) is a clinical leader with responsibility for the daily operations of the residential hospice, Campbell House, ensuring that the care provided follows the philosophy and goals of the organization.
The CSC initiates assessments on patients prior to admission, works with the admitting nurse to develop a collaborative care plan with patient, family (if desired by patient), staff and physicians, directs the activities of the residential hospice staff and volunteers with a focus of providing excellent hospice palliative care to patients and families. Works collaboratively with the HGT Integrated Care Team as well as physicians and community partners to strengthen and develop HGT’s role in the provision of excellent palliative care in the South Georgian Bay region.
The CSC is a key member of the Leadership Team and is responsible for high standards of patient care, efficient resource management, and for participating in short and long term planning for the organization.
In collaboration with the Executive Director, the CSC ensures Campbell House is compliant with provincial standards and regulations with regards worker health and safety, and compliant with local and provincial standards for patient safety, fire prevention, food handling and waste management.
